Our last post covered the inpatient side of Precision Cardiac Amyloid — the Medicare New Technology Add-on Payment (NTAP) effective October 1, 2026. Most echocardiograms, however, are not inpatient studies. This post covers the workflow for navigating CPT 0932T, the outpatient AI code, which has been active since January 1, 2025.
Many medical coders might not recognize that cardiac amyloidosis is an increasingly recognized form of heart failure. As misfolded protein deposits into the heart, the heart walls thicken and stiffen — leading to restrictive cardiomyopathy and heart failure. The 0932T AI echo code is colloquially described as a heart failure code; cardiac amyloid, however, was considered and intended for this CPT code. In fact, the underlying academic literature behind Precision Cardiac Amyloid (Duffy et al., JAMA Cardiology 2022)1 was a key part of the supporting literature behind the AMA approval of this CPT code, and the AMA CPT Editorial process specifically considered cardiac amyloidosis in its decision-making.
1. What the code says
Here is CPT 0932T in full, as published:
0932T — Noninvasive detection of heart failure derived from augmentative analysis of an echocardiogram that demonstrated preserved ejection fraction, with interpretation and report by a physician or other qualified health care professional.
— AMA CPT® Category III code 0932T, effective January 1, 20252
Three things in that sentence do the work, and it is worth reading them separately.
"Noninvasive detection of heart failure." The code describes the detection of heart failure. It does not name a mechanism, an etiology, or a phenotype. An earlier draft of the CPT code referenced heart failure with preserved ejection fraction (HFpEF), but that was intentionally taken out to capture all forms of heart failure, and not just one diagnosis.
"Derived from augmentative analysis of an echocardiogram that demonstrated preserved ejection fraction." The preserved-EF condition attaches to the echocardiogram, not to the diagnosis. To bill this code, the underlying echocardiogram study needs an LVEF above 50%, and the AI analysis must detect some form of heart failure. If InVision Precision Cardiac Amyloid is run and the LVEF is below 50%, use CPT code 93799.
"With interpretation and report by a physician or other qualified health care professional." Algorithms in this space are diagnostic aids, meant to assist and augment the clinician diagnosis and not to be the sole determinant of clinical decision-making. In patients with very high or very low pre-test probability, clinician judgment should override algorithm input and guide clinical decision-making.
Unlike the CMS NTAP documentation, the CPT announcement does not name a particular product. The descriptor contains no manufacturer name, product name, or trade name; CPT codes describe a type of service. The AMA CPT Editorial Panel that publishes them is explicitly payment-blind, coverage-blind, and vendor-blind, and Category III codes are written to describe an emerging service that can be tracked across everyone who performs it.
2. Cardiac amyloidosis is a treatable form of heart failure
Cardiac amyloidosis is an infiltrative cardiomyopathy, most commonly from deposition of TTR protein or light chains. Amyloid fibrils deposit in the myocardium, the walls thicken, the ventricle stiffens, and filling pressures rise — leading to atrial fibrillation, volume retention, autonomic dysfunction, and heart failure.
Because the process is restrictive — the heart muscle stiffens and has impaired relaxation — ejection fraction is typically preserved until late in the disease. Because the LVEF looks normal, the study can be read as normal or as HFpEF, and the patient never undergoes additional testing for cardiac amyloidosis.
With the ability to identify patients often before they are clinically recognized,3 Precision Cardiac Amyloid flags studies at high suspicion of AL or ATTR cardiac amyloidosis.
3. The clinical evidence for 0932T
Category III codes are assigned with the support of clinical evidence. The Editorial Panel requires peer-reviewed literature demonstrating the clinical efficacy of the service, and the code is established on the strength of that evidence.
The supporting literature submitted with the code application for 0932T included Duffy et al., High-Throughput Precision Phenotyping of Left Ventricular Hypertrophy With Cardiovascular Deep Learning, JAMA Cardiology 2022;7(4):386–395 (doi:10.1001/jamacardio.2021.6059)1 — the study known in the literature as EchoNet-LVH, which became the foundation for InVision Precision Cardiac Amyloid, cleared by the FDA as 510(k) K243866 on May 21, 2025. The algorithms described included AI for detecting cardiac amyloidosis, hypertrophic cardiomyopathy, and valvular cardiomyopathies. This paper does not have a HFpEF classifier.
4. Where 0932T sits in the AMA's AI taxonomy
The AMA classifies AI-enabled services as assistive, augmentative, or autonomous. 0932T is an augmentative service: the software analyzes the study and produces a clinically meaningful output, and a physician interprets that output and reports it.
That classification is consistent with how InVision Precision Cardiac Amyloid is cleared and how it is used. The algorithm runs in-workflow on the echocardiogram being performed, returns a result alongside the study, and the reading cardiologist interprets it as part of the study read. The device is adjunctive. The physician makes the diagnosis.
Most of the cardiac imaging AI codes in current use — 0932T, 75577, 75580 — sit in the same place for the same reason.
5. How to report it
0932T is reported in addition to the echocardiogram. The echocardiogram acquisition codes (93306, 93307, 93308, 93350, and 93351) are billed exactly as they are today. The AI analysis carries its own code, in the same way strain imaging or 3D acquisition is reported alongside the study.
| Setting | Report | Rate |
|---|---|---|
| Hospital outpatient — technical component | 0932T | $313.79 national average, wage-adjusted |
| Physician professional — any place of service | 0932T | Carrier-priced |
| Office / IDTF — place of service 11 | 0932T | Carrier-priced |
In the hospital outpatient setting, 0932T carries status indicator S under OPPS — separately payable, not packaged into the echocardiogram.4 Under the Physician Fee Schedule, Category III codes are contractor-priced, so the professional and office rates are set locally rather than published nationally.
Report 0932T when all of the following are true: the echocardiogram demonstrated preserved ejection fraction, an augmentative artificial intelligence analysis was performed, and a physician or other qualified health care professional interpreted the output and documented it in the report.
6. Documentation
The physician's report should reflect that the augmentative analysis was performed and that its output was reviewed as part of the interpretation. This is the same workflow that applies to strain, 3D, or contrast. Most sites handle this in the existing echo order set with a checkbox for when the AI echo analysis is performed. It requires no separate encounter and no new workflow step.
7. When the outpatient code does not apply
If the echocardiogram demonstrates an LVEF below 50%, 0932T does not apply. The descriptor is explicit, and there is no reading of it that gets you there. The New Technology Add-on Payment that CMS approved for InVision Precision Cardiac Amyloid — identified on the claim by ICD-10-PCS code XEZZXLC, effective October 1, 2026 — carries no ejection fraction qualifier and would apply irrespective of LVEF range.
8. What it pays, and what changes in January
For CY2026, 0932T pays a national average of $313.79 in the hospital outpatient setting — APC 5743, relative weight 3.4326, wage-adjusted to your locality.4 In CY2027, the proposed rule would move 0932T onto a new status indicator O1 and APC 1505 at $350.50, an increase of roughly 11.7 percent.5 That is a proposed rule. It is not final, and it will not be until CMS publishes the CY2027 OPPS final rule in November.
The short version
0932T requires that the echocardiogram demonstrated preserved ejection fraction, that augmentative software analyzed it, and that a physician interpreted and reported the result. Cardiac amyloidosis is an infiltrative cardiomyopathy that produces heart failure and increased wall thickness — and the evidence for cardiac amyloidosis formed the foundation of the 0932T CPT code.
